Insurance providers make changes daily to benefits, therefore, your carriers have made it impossible for us to know your plan provisions, benefits, payments, coverage, and whom you may or may not see. Please be aware that insurance was designed to be used as a reimbursement for payment, not as a substitute for payment. As a courtesy to you, we file your primary insurance and only require that the estimated, uncovered portion be due at the time services are rendered. All information necessary to file subsequent or secondary insurance will be provided to you so that you may be reimbursed.
Your dental benefit may vary for a number of reasons:
-You have already used some or all of the benefits available from your dental insurance.
-Your insurance plan paid only a percentage of the fee.
-The treatment you needed was not a covered benefit.
-You have not yet met your deductible.
-You have not reached the end of your plan’s waiting period and are currently ineligible for coverage.

Once payment is made, usually in four to six weeks, any adjustments due you or remaining balance will be settled or due immediately. Most dental insurance carriers cover endodontic procedures at a rate that varies from 50% to 80% of their established Usual Customary Rate fee in the Brentwood/ Columbia, TN area.
